It is necessary to determine the level of anxiety within the depression, for it may be anxiety predominant. Considering the overall very low compliance, a dimensional approach is required.

If medication nieve, generally start with:
Sertraline 25mg
Trazadone 25 mg-50mg ( antagonism at 5ht2c can prevent some some side effects from the ssri)
clonazepam .5-1 mg prior to sleep. ( With Prozac a significant increase in time to improvement, improves tolerability, relieves insomnia)

All used at night. If pt has a history of successful use the start 50sertraline, 50 Trazadone, 1 mg clonazepam all at night.
