Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia characterized by loss of muscle atonia during REM sleep that results in motor behaviors. Diagnosis of RBD involves a clinical interview in which history of dream enactment behaviors is elicited and a subsequent overnight polysomnography (PSG) evaluation to assess for REM sleep without atonia (RWA) and/or observe motor behaviors during REM sleep. Recurrent nocturnal behavioural and movement paroxysms are a diagnostic challenge for the clinical pediatrician. We report on an adolescent girl who presents recurrent stereotypical nightmare-like episodes occurring during non-REM sleep stages 1-2 (N1 and N2). We discuss the differential diagnoses between epileptic and nonepileptic events and between nocturnal frontal and temporal seizures. The pathophysiological and unusual electroencephalographical features are discussed with respect to clinical features and results of interictal FDG-PET. Conclusion In case of stereotypical nightmare-like episodes in children or adolescents, an epileptic origin has to be ruled out before a parasomnia is diagnosed. In addition, a normal awake EEG or interictal sleep EEG in the diagnostic workup may not exclude an epileptic disorder. In case of nightly stereotypic motor or affective events, an epileptic disorder should be discussed.

REM sleep behavior disorder is a unique parasomnia characterized by dream enactment behavior during REM sleep. Unless triggered by pharmacologic agents such as antidepressants, it is generally related to damage of pontomedullary brainstem structures. Idiopathic REM sleep behavior disorder (RBD) is a well-established risk factor for neurodegenerative disease. Prospective studies have estimated that at least 40-65% of patients with idiopathic RBD will eventually develop a defined neurodegenerative phenotype, almost always a 'synucleinopathy' (Parkinson's disease, Lewy Body dementia or multiple system atrophy). In most cases, patients appear to develop a syndrome with overlapping features of both Parkinson's disease and Lewy body dementia. Idiopathic rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia that is an important risk factor for Parkinson's disease (PD) and Lewy body dementia. Its prevalence is unknown. One barrier to determining prevalence is that current screening tools are too long for large-scale epidemiologic surveys. Therefore, we designed the REM Sleep Behavior Disorder Single-Question Screen (RBD1Q), a screening question for dream enactment with a simple yes/no response.|Four hundred and eighty-four sleep-clinic-based participants (242 idiopathic RBD patients and 242 controls) completed the screen during a multicenter case-control study. All participants underwent a polysomnogram to define gold-standard diagnosis according to standard criteria.|We found a sensitivity of 93.8% and a specificity of 87.2%. Sensitivity and specificity were similar in healthy volunteers, compared to controls or patients with other sleep diagnoses.|A single-question screen for RBD may reliably detect disease
