Our study aims to make inferences from inpatient admission of conversiondisorder with regards it's age and sex distribution, clinical presentation, yearly distribution of admission and morbidity of conversiondisorder in comparison to total psychiatric admissions. It also compares it's results with those of previous studies in this country, neighbouring countries and western studies.|A case notes review was done for admissions of this disorder for five years in Mosul University Hospital. The data were inputted into SPSS programme and analysed. The statistical analysis was by t-test, Anova and regression.|Two hundred seventy five patients were admitted during the period; 224 women and 51 men. The majority of men came from Urban areas compared to 58% of women. Single status was over represented; 59% compared to 34% married. Conversiondisorder is largely managed by neurologists, for whom it presents great challenges to understanding and management. This study aimed to quantify these challenges, examining how neurologists understand conversiondisorder, and what they tell their patients.|A postal survey of all consultant neurologists in the UK registered with the Association of British Neurologists.|349 of 591 practising consultant neurologists completed the survey. They saw conversiondisorder commonly. While they endorsed psychological models for conversion, they diagnosed it according to features of the clinical presentation, most importantly inconsistency and abnormal illness behaviour. Most of the respondents saw feigning as entangled with conversiondisorder, with a minority seeing one as a variant of the other. They were quite willing to discuss psychological factors as long as the patient was receptive but were generally unwilling to discuss feigning even though they saw it as their responsibility.
Despite the avalanche of empirical data on prodromal/"at risk" conditions, the essential aspects of the vulnerability to the schizophrenia spectrum remain largely unaddressed. We report here the results of the Copenhagen Schizophrenia Prodromal Study, a prospective, observational study of first admission patients in putative state of beginning psychosis (N=151) with a follow-up length of 60 months. At follow-up, the rate of conversion to schizophrenia spectrum diagnosis was 37%, whereas the conversion rate from schizotypal disorder to schizophrenia was 25%. High levels of perplexity and self-disorders baseline scores yielded the best prediction of the subsequent development of schizophrenia spectrum disorders. Escalating transitions within the spectrum (i.e., from schizotypal disorder to schizophrenia) were not associated to any candidate psychopathological predictor.
Disproportionate hyperproinsulinemia is an indicator of -cell dysfunction in diabetes and the basis underlying this abnormality remains obscure. Recently, we have found proinsulin is an aggregation-prone molecule inherent with a low relative folding rate and maintains a homeostatic balance of natively and plentiful non-natively folded states (i.e., proinsulin homeostasis, PIHO) in normal -cells as a result of the integration of maturation and disposal processes. PIHO is susceptible to environmental and genetic influences. Perturbation of PIHO produces a number of toxic consequences with known association to -cell failure in diabetes. To explore whether the perturbation of PIHO has a link to disproportionate hyperproinsulinemia, we investigated proinsulin conversion and the involved prohormone convertase 1/3 (PC1/3) and 2 (PC2) in mouse Ins2(+/Akita) islets/-cells that preserve a primary PIHO disorder due to a mutation (C96Y) in the insulin 2 (Ins2) gene.
